How to Apply Medical Leave?

  1. Proceed to Leave> Application> Medical Leave
  2. User able to see their MC Taken on top of the form before applying
  3. Fill in the form to apply leave
  4. Two types of medical leave are accessible which are MC Leave, authorized by a doctor and Hospitalized, necessitated when we are admitted to the hospital due to illness.
  5. User also able to upload file, such as medical certificate (MC) as evidence which must be included within the system when applying medical leave
  6. Then, click the “Next” button
  7. After ensuring all data is entered correctly, the user can proceed to submit the form and simply await approval from the PIC for the leave request
