How to apply for medical leave?

1. Proceed to Leave > Application > Medical Leave
2. User able to see their MC Taken on top of the form before applying
3. Fill in the form to apply leave
4. Two types of medical leave are accessible 
   1. MC Leave: Authorised by a doctor 
   2. Hospitalised: For when a user is admitted to the hospital due to illness
5. User also able to upload file, such as medical certificate (MC) as evidence, which must be included within the system when applying for medical leave
6. Click Next button
7. After ensuring all data is entered correctly, user can proceed to submit the form and await approval from the PIC for the leave request
